Musculoskeletal physiotherapy

We treat the load, not just the sore spot

Hollow Thread is a small practice for stubborn joints, tendons and backs. Every case starts with a 60-minute assessment, then a written plan you can actually follow between visits.

  • 60-minute first visit
  • Hands-on plus loading
  • Written plan after every session
  • Self-referral welcome

Start where it hurts

Six lanes we see most weeks

Find the closest match to your problem. Each lane lists what gets tested in the first visit and what the early work usually looks like, so you know what you are booking.

Lower back & sciatic pain

Lifting · sitting · morning stiffness
First visit tests
Hip and spine movement under load, nerve tension, and how symptoms behave with repeated bending.
Early work
Positions that settle the nerve, then hinge and carry patterns rebuilt from a weight you can control.

Knee & patellar tendon

Stairs · running · post-surgical
First visit tests
Single-leg control, calf and quadriceps capacity, plus how the tendon reacts the day after loading.
Early work
Isometric holds for pain relief, then a slow build through step-downs and split-stance strength.

Shoulder & rotator cuff

Overhead · night pain · frozen shoulder
First visit tests
Range in and out of rotation, cuff strength at several angles, and how the neck contributes.
Early work
Hands-on release where range is limited, paired with cuff loading you repeat on non-clinic days.

Neck, jaw & desk headaches

Screen work · clenching · tension
First visit tests
Neck segment mobility, deep neck endurance, jaw opening, and the actual layout of your workstation.
Early work
Manual therapy for the stiff segments, breath and jaw downtraining, and two desk changes that stick.

Hip, groin & pelvis

Field sport · postnatal · long walks
First visit tests
Adductor and abductor strength, rotation range, and tolerance to change of direction or stairs.
Early work
Groin loading at short range first, then lateral strength and gradual return to cutting movements.

Foot, ankle & Achilles

Sprains · plantar heel · calf tears
First visit tests
Calf endurance to fatigue, ankle bend depth, balance under load, and current footwear.
Early work
Heel raise volume built week by week, plus hopping and landing drills once pain settles below a threshold.

The pathway

Four stages, one thread

Most people move through these stages rather than attending open-ended weekly visits. You will know which stage you are in and what ends it.

Stage 01

Assess

A full hour: history, testing, an explanation you can repeat to someone else, and a written plan before you leave the room.

Visit one · 60 minutes

Stage 02

Settle

Reduce the irritability with hands-on work, load adjustments and a short daily routine that fits around your week.

Usually 1–3 visits

Stage 03

Rebuild

Progressive strength on the tissue that failed, retested in clinic so the jump in load is measured, not guessed.

Fortnightly reviews

Stage 04

Return

Put the demand back in — sport, shift work, lifting or travel — and agree the maintenance work that keeps it there.

Discharge plus one check-in

Reconditioning floor with barbells, benches and open space for loaded rehabilitation
Reconditioning floor — used in most rebuild sessions
Therapist performing hands-on soft tissue treatment on a patient's shoulder in a quiet treatment room
Private treatment room — hands-on and quiet testing

Practical detail

What a visit is actually like

Two rooms and one open floor. Sessions run on time, and nobody is left on a machine while the clinician works elsewhere.

  • Session length

    60 minutes for a first assessment, 30 or 45 minutes for follow-ups.

  • What to wear

Loose shorts or leggings, plus trainers if your problem involves walking, running or lifting.

  • Bring with you

Any scan reports or surgical notes you have, and the shoes or kit you use most.

  • Who you see

The same clinician through your pathway — Marin Adeyi for spine and tendon work, Rosa Kelleher for post-surgical and sport reconditioning.

  • Between visits

Your plan is emailed as a short list with sets, reps and the pain level to stay under.

  • Open hours

Monday to Friday, early mornings to evening, plus Saturday mornings.

Before you book

Questions we get asked

Straight answers to the six things people email about most.

Do I need a referral from a doctor?

No. You can book directly. If something in your assessment suggests it needs medical imaging or a specialist opinion, we will tell you in that first visit and write to your doctor with your permission.

Will the first session make me sore?

Some stiffness for a day is normal after testing, especially with tendon and back problems. It should settle within 24 hours. We set a pain ceiling with you before loading anything, and the plan is adjusted if you go over it.

Can you provide paperwork for insurance or work?

Yes. Itemised receipts are issued after each visit, and we can write a short progress letter for an insurer, employer or occupational health team on request. Check what your policy needs before the first visit so we include the right details.

How much exercise will I have to do at home?

Usually two or three movements, ten to twenty minutes, three to five days a week. If a plan does not fit your actual week we shorten it rather than hope. Consistency at a lower dose beats an ambitious plan you abandon.

What if I need to move or cancel an appointment?

Let us know at least 24 hours ahead and we will move it at no cost. Late cancellations may be charged, because the slot is held for you. Shift workers can ask for a standing early or late time.

How long until I can play sport again?

It depends on the tissue and the sport, not the calendar. We use strength and hop or landing tests against your other side, and agree the criteria at your first visit so return is a decision you can see coming rather than a guess.
